Job Description:
Job Title: Case Manager – Populance Location: Detroit, MI (Local
candidates preferred) Pay Rate: $38/hour Schedule: Monday–Friday,
8:00 AM – 5:00 PM Overview: Seeking an experienced and
compassionate Case Manager (RN or LMSW) to join the Populance Case
Management Team. This is supporting ambulatory care services
through telephonic case management and care coordination. Benefits:

Enroll patients into case management programs via telephonic
outreach · Assign appropriate billing codes to services (training
provided if needed) · Provide patient-centered care planning and
coordination · Conduct assessments focused on Social Determinants
of Health (SDoH) · Collaborate with interdisciplinary teams to
ensure quality outcomes · Document care in Epic’s Compass Rose
system (training available) · Meet performance metrics in a
fast-paced, high-volume environment
