Job Description:
Kaiser Permanente Georgia is seeking a dedicated and
compassionate Patient Care Coordinator RN to join our team at Holy

candidate will be a beacon of support, helping to harmonize
treatment plans with patient wellness goals, while striving to
provide both care and coverage seamlessly. Assess and evaluate
patient needs and develop appropriate healthcare plans. Coordinate
with doctors, nurses, and other healthcare professionals to ensure
cohesive care delivery. Communicate effectively with patients and
their families, providing clear information on healthcare options
and procedures. Manage patient records and ensure confidentiality
and accuracy in documentation. Participate in patient advocacy and
maintain high standards of care. Strong communication and
interpersonal skills Expertise in clinical practices and healthcare
coordination Ability to handle stress and make quick decisions
