Job Description:

Provide quality nursing care to residents; implement specific procedures and programs; coordinate work within the department, as well as with other departments; report pertinent information to the immediate supervisor; respond to inquiries or requests for information; assist the immediate supervisor with tasks to support department operations.Provide quality nursing care to residents in an environment which promotes their rights, dignity, and person centered care including working directly with residents as necessaryProvide individualized attention, which encourages each resident's ability to maintain or attain the highest level of independence and physical, mental, and psychosocial well-beingDisplays high regard for personal dignity, diversity, and uniqueness of individuals (cultural/religious sensitivity, confidentiality, privacy, age specific competencies)Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies such as lighting or equipment problems to the supervisor or appropriate staff Protect the personal belongings of each resident. Promptly report missing items according to established policy and participate in efforts to locate missing itemsFormulates effective working relationships and communication with staff, patients, families and other customersCommunicate and interact effectively and tactfully with the resident, visitors, families, peers, and supervisorsPerform all job responsibilities in accordance with prescribed safety and standard precautionsContributes to the success of the facility including employee engagement.Supports the DRC and Wellspring management in clinical and operational needsParticipate in the Quality Assurance and Performance Improvement committee, and assist in developing and implementing appropriate plans of action to improve performanceEvaluate outcome of care and nursing judgments on a daily basis to assure high quality care Able to communicate with staff and monitor for any problems; follow up with DRC if necessary and proceed with disciplinary steps as needed per policyComplete evaluations for nursing staff on the unit as directed Assure the aides are completing walking rounds and report to each other at shift change; assure that the nurse gives a thorough report to the aides; assure that the off-going nurse gives a thorough report to oncoming nurse Review EHR periodically to assure proper and timely documentation being done Assure documentation reflects the resident’s status, indicating progress, maintenance, or decline; must ensure that a resident’s abilities in activities of daily living do not diminish unless circumstances of the resident’s clinical condition demonstrate that decline was unavoidable Responsible to assure incident reports are filled out when needed, and properly done; assist DRC with instruction when areas are incomplete or not accurately assessed Document and review on 24 hr. report daily. Identify sheets any pertinent issues and/or changes; assure that each shift follows through with needed documentation. Ensure physician and family are notified as per policyFollows up with staff when necessary to assure compliance to standards ofdocumentationMaintains high regard for confidentialityAdheres to budgetary practicesAttends/participates in departmental meetings and care conferencesParticipates in the “twenty-four (24) hour “on-call” rotationHas direct responsibility for one or more areas, which may include:Staff Development and Infection ControlCollects an analyses data for diverse sources in order to problem solve for appropriate decision makingDemonstrates self-awareness of self-responsibility and accountability for own actionsSeeks validation of knowledge base, skill level, decision making as necessary and assertively seeks guidance in area of questionMaintains professional standards by actively participating in continuing education activities, in-service educationActively participates in orientation, supervision, training and evaluation ofstudents and other staffAssures compliance and readiness for emergency preparedness with campus leadershipObserves, monitors and evaluates outcomes, customer satisfaction and resident rights integration. Suggests interventions to enhance resident rights.Care Plan Responsibilities:Assists DRC or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes.Ensures that a care plan is initiated on every patient upon admission to the unitActive role with pre-admission process and resident admissionsStaff Development/Infection Control ResponsibilitiesParticipates in the management of select direct and indirect patient care activities for complex, atypical patients and familiesEvaluates patient care practices, integrating research based therapeutics and continuous quality improvement to enhance outcomesDocuments all patient care observations and interventions in the electronic record accurately, clearly, in a timely manner following all established policiesMaintains professional development through seminars, workshops, in-service activities, and current professional literatureCoordinates, develops and evaluates patient and family education for effectiveness and cultural appropriateness with input from physicians and other expertsIdentifies and develops staff educational activities to promote professional growth and competent practiceRecommends proposed therapeutics (drugs, equipment, products) based on an analysis of cost-effectiveness, clinical efficacy, and efficiencyPromotes a positive work environment through professional conduct, teamwork and a helpful, positive attitude to all co-workers and customersSupports and promotes resident and client rights and allows for privacy, dignity, and well-being of allSupports and promotes safety, infection control, quality assurance and compliance with all organization and regulatory requirements; State and FederalAdheres to all organization and site specific policies and proceduresComplies with current law and policy to provide a work environment free from intimidation, bullying, harassment, sexual harassment and all illegal and discriminatory behaviorParticipate in meetings as required Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstancesParticipate in staff meetings and education programs designated for attendancePerforms other duties and achieves other results as assignedMust be currently licensed in Michigan (in good standing) as either a Registered Nurse or Licensed Practical NurseTwo years of nursing experience with current long term or sub-acute care, medical, surgical, or critical care experience preferred**Wellspring Lutheran Services, Infection Control, Assisted Living, RN, LPN, Saginaw*
