[ "The Quality Safety Value Coordinator (QSVC) is organizationally responsible to the Chief of Quality Safety Value Services (QSV) who thereby reports to the Medical Center Director. The QSVC performs analytical, evaluative, developmental and advisory work specific to multiple accreditation guidelines and regulations, specifically the Joint Commission (Ambulatory Care and Behavioral Standards) and Commission on Accreditation of Rehabilitation Facilities (CARF), Office of the Inspector General (OIG) reviews, Office of Mental Health Operations (OMHO), and numerous laws and regulations governing the QSV service. The QSVC is responsible for the appropriate interpretation of the OIG, The Joint Commission standards for ambulatory care/outpatient activities, The Joint Commission standards for mental and behavioral health and CARF standards for behavioral health survey activities. New standards may become applicable as new programs are introduced. The level of standards evaluation, analysis of compliance, development of action plans and standard-specific monitoring and evaluation have major impact on the organization's accreditation status. The QSVC is responsible for development of strategic and tactical plans related to compliance with, and improvement of, all applicable standards and elements of performance. The QSVC provides direct staff support to top management, Service Chiefs, and others in planning, designing, integrating, implementing, modifying, and administering an effective Quality Assessment and Improvement Program to: (1) Evaluate the quality and safety of patient care delivery\n(2) Improve the quality and safety of patient care delivery at the Community Based Outpatient Clinics (CBOCs) and the Saginaw Annex\n(3) Evaluate the appropriateness and timeliness of services provided. Duties include but are not limited to: Joint Commission (TJC) Team Facilitator (J Team)- The primary function of the QSV Coordinator is to act as technical advisor, on matters relating to organizational accreditation, for all levels of Behavioral Health -Medical Center management and staff. The role of technical advisor encompasses the following minimum duties: Provides accurate and timely interpretation of TJC and CARF standards. The QSVC's knowledge base is maintained through a variety of mechanisms, to include mentoring, self-study, educational seminars, telephonic conferences, and networking with other QSV professionals and organizations. When complex standard interpretation is required, the QSVC shall correspond with TJC's Standards Interpretation Group (SIG) and obtain formal interpretation in writing. o Ensures facilitation and coordinator of CARF programs across the organization, including manuals and standards review, compliance, finding mitigation, and survey coordination. Serves as a liaison between external CARF contacts and internal CARF program leads. o Advises the TJC Team Leader on the general assessment process for the examination of compliance with standards. Direction is given on policy review, performance monitoring, and systems analysis. The QSVC technical advisor is to be considered the organizational subject expert for the Behavioral Health TJC, CARF and OIG requirements for compliance, and provides final and definitive interpretation of standards for the team. o Provides Just in Time training to team leader/members. Facilitates understanding of the goals and outcomes to be achieved per Joint Commission Standards Compliance Team and Performance Improvement (PI) approaches/tools to be used in the assessment process. Methodology (process used to analyze an organizations system, specific area or process or attention to identified priority focus areas in order to achieve continuous survey readiness) o Assists in the identification of data sources to be examined by the team, utilizes and assists team to perform Tracers; reports analysis of tracers to leadership monthly. o Validates findings of the J-Team Tracers through a variety of analytical techniques. Examples include the comparison of policy language with discrete wording from the standards/elements of performance, verification of compliance with action plan/policies/procedures at the Service level, and verification of statistics and data presented by the team in the assessment process.
