Job Description:
$6,000 Recruitment Bonus for Clinical Master level new hires!!
($2,000 paid at start, $2,000 paid after 3 months, and $2,000 paid
after successful probation period.) Under the supervision of the
Mental Health Supervisor (Wraparound Services). This position seeks
to actively identify appropriate referrals and proceed with
referrals for screening and assessment of children/youth that may
be eligible for the Serious Emotional Disturbance (SED) Waiver, or
other mental health services. This position is responsible for the
completion of the SED Waiver application process and tracks service
intensity for all Wraparound consumers. This position works closely
with the finance department to reconcile DCH SED Waiver payments.
This position may provide some services at non-SCCMHA facility
(i.e. Saginaw County DHHS, community service agencies) as needed in
an effort to increase the appropriate use of the SED Waiver for
children/youth in Saginaw County. This position must be available
for consult on any child/youth served by DHHS and Juvenile Justice
who may require mental health services. This position may provide
clinical support to the Wraparound team. This position will be
knowledgeable about and actively support culturally competent
recovery-based practices; person centered planning as a shared
decision-making process with the individual, who defines his/her
life goals and is assisted in developing a unique path toward those
goals; and a trauma informed culture of safety to aid consumer in
the recovery process. Job Requirements: Act as the Children's
Department liaison for all potential SED Waiver consumers through
participation in the Wraparound Orientation Team Meetings to
explain the Waiver and its benefits to the families served. -
Follow guidelines for successful, and timely completion of the SED
Waiver application and reporting process in coordination with other
CMHSP staff Utilize the SED Waiver Application Checklist and other
quality assurance measures to guarantee that submitted SED Waiver
applications are accurate and complete. Ensures that required SED
paperwork is submitted according to state standards. Complete
and/or compile the information to be submitted as part of the SED
Waiver application packet which includes Cover Letter, Waiver
Certification, Children's SED Waiver Annual Budget, Demographic
Intake Data, Family Choice Assurance, CAFAS Summary and a copy of
Medicaid Card (if currently eligible). - Works collaboratively with
the state's SED Waiver Coordinator ensuring any changes, and
updates are communicated to the supervisor and department director
for certify compliance. - May complete a full intake assessment
with a child/youth and family when the child/youth is not eligible
for the SED Waiver, but is eligible for other CMHSP services. This
intake assessment must be sufficient so that the child/youth and
family are not required to go through the access/intake process at
the CMHSP again to access mental health services. - Maintains a
collaborative partnership with the local DHHS office to assist with
coordination and identification of appropriate referrals for mental
health services, SED Waiver applications, including serving as a
liaison and assist DHHS staff with necessary reports/documentation
for court. Provide training and technical assistance for DHHS,
Juvenile Justice as well as other child/youth serving community
agencies regarding the mental health needs of children and
families, the availability of mental health services, and how to
access to such services. Be available for consult on any
child/youth served by DHHS who may require mental health services.
- Based on the needs of the Department may provide direct mental
health services for youth and families. -Formulates individual
plans of service designed for specific symptoms, completes
assessments, monitors progress of service and treatment from
individual, family, and group therapy sessions, completes quarterly
reviews and considers treatment options. Provide referrals to
mental health services outside the CMHSP, if the child/youth does
not qualify for community mental health services. Participate in
permanency planning meetings, residential review (DHHS), and Team
Decision Making (DHHS). Active participant of the Wraparound
Community Team who is responsible for maintaining documentation of
participants, referrals, and outcomes of the meeting. - Monitors
all Wraparound consumer records and tracks service intensity to
ensure that all Wraparound cases are meeting the state
requirements. Works closely with the finance department to
reconcile DCH SED Waivers. - Participate in state-level training,
3-day MDCH Wraparound Facilitation training, and technical
assistance regarding the SED Waiver DHHS Pilot Projects. Adheres to
the mission, vision, core values and operating principles of SCCMHA
at all times
