The Case Manager coordinates all services necessary to maximize
consumer recovery and independence for individuals with mental
illness including implementing strategies for obtaining services

Management - Ensure consumer involvement in the Person Centered
Planning Process (PCP). - Facilitate the implementation of
Individual Plan of Service. - Perform comprehensive, ongoing
psychosocial assessments. - Participate in legal proceedings
including petitioning and testifying in clinical emergencies. -
Provide community based services. - Facilitate the coordination of
consumer intakes, discharges, and transfers. - Communicate with all
service providers in Treatment Plan. - Facilitate hospitalizations,
attend hospital meetings and coordinate with hospital social
worker. - Coordinate and monitor activities such as a job, school,
volunteerism, and family caretaking. - Provide referral to
appropriate community resources. About you… - You are passionate

for others. You have… - Bachelor’s degree in a human services field
from an accredited institution required. - LLBSW, LBSW, LLMSW, LMSW
preferred. - Prior case management experience in a mental health or
rehabilitation setting preferred. - Valid Michigan Driver’s License
required. Education Required Bachelors or better Bachelor of Arts
or better in Criminal Justice Bachelor of Social Work or better
Bachelor of Human Services or better Bachelor of Science or better
in Psychology Bachelor of Science or better in Criminal Justice
Licenses & Certifications Required - Driver's License Preferred Lic
